Neuroborreliosis: diagnostic problem in distinguishing from multiple sclerosis
نویسنده
چکیده
Objectives: To present the importance of additional diagnostic procedures in diff erential diagnosis of multiple sclerosis and neuroborreliosis, exemplifi ed by a case of our patient. Neurological manifestation of Lyme disease varies and sometimes can imitate multiple sclerosis, especially in the early disseminated or late phase when there are no data on clinical infection. Case report: We present a patient with neuroborreliosis, initially considered as multiple sclerosis. Positive serology for Borrelia burgdorferi is an indicator of a past infection. However, the presence of specifi c IgG and IgM antibodies in the CSF confi rms the diagnosis. In our case, magnetic resonance imaging (MRI) of the brain showed normal fi ndings, whereas during spinal cord MRI, edema and hyperintense lesions of the thoracic spinal cord were found. Visually evoked potentials revealed prolonged latencies of the P wave on the left side. CSF analysis showed proteinorrhachia and distinct pleocytosis, with positive serology for specifi c antibodies to Borrelia burgdorferi. Conclusion: Th e clinical symptoms, MRI and CSF fi ndings in combination with good response to antibiotic therapy confi rmed the diagnosis of neuroborreliosis.

MR imaging assessment of brain and cervical cord damage in patients with neuroborreliosis.
BACKGROUND AND PURPOSE Neuroborreliosis is frequently indistinguishable from multiple sclerosis (MS) on both clinical and radiologic grounds. By using MR imaging, we assessed "occult" brain white matter (WM), brain gray matter (GM), and cervical cord damage in patients with neuroborreliosis in an attempt to achieve a more accurate picture of tissue damage in these patients, which might contribu...
متن کاملMultiple sclerosis and Lyme borreliosis.
In a deductive approach the two disease entities of multiple sclerosis and chronic progressive neuroborreliosis are discussed. Various clinical features, seroepidemiology, neuroimaging, CSF findings, CSF serology, specific proteins within the CSF and antibodies against neuronal structures as well as the most recent findings of different dendritic cells within the CSF are discussed as a means of...
متن کاملBeta2-Microglobulin as a Diagnostic Marker in Cerebrospinal Fluid: A Follow-Up Study
Beta2-Microglobulin ( β 2-m) is a low molecular weight protein occurring in all body fluids. Its concentration increases in various pathologies. Increased values in cerebrospinal fluid (CSF) are ascribed to an activation of immune system. Using immunoturbidimetry, we examined concentrations of beta2-microglobulin in cerebrospinal fluid in a large group of 6274 patients with defined neurological...
متن کاملA challenging entity: multiple sclerosis or collagen tissue disorders: A case series of 6 patients
Background: Multiple sclerosis and other demyelinating processes are sometimes difficult to differentiate from the neurological involvement in autoimmune diseases. Distinguishing multiple sclerosis from other lesions due to autoimmune diseases is crucial to avoid unsuitable or delayed treatments. Methods: Charts of 6 patients diagnosed with mimicking multiple sclerosis between 1996 and 2014 we...
متن کاملA Case of Relapsing-Remitting Neuroborreliosis? Challenges in the Differential Diagnosis of Recurrent Myelitis
We report the case of a 31-year-old woman with 4 episodes of myelitis with pleocytosis, a positive Borrelia burgdorferi serology with positive antibody indices, and full recovery each time after antibiotic and steroid treatment, suggesting neuroborreliosis. We nevertheless believe that recurrent neuroborreliosis is improbable based on the levels of the chemokine CXCL13 in cerebrospinal fluid an...
